I acknowledge that I have read and understand the following description of Global Liver Institute’s use of personal information, likeness, and related materials (“Attributes”), and I indicate my choice regarding permission at the end of this document.
Global Liver Institute and its affiliates, successors, and assigns may seek to copyright and/or publish, reproduce, distribute, or otherwise use my name (as imputed in this registration form ), voice, likeness, and/or any other characteristic about or concerning me (“Attributes”) in video, photographs, written materials, and audio-visual recordings, and any materials created (“Copyright Materials”) by or on behalf of Global Liver Institute throughout the universe in any medium or format whatsoever now existing or hereafter created. These materials may be used for both commercial and/or non-commercial purposes.
If permitted, my Attributes may be edited, reproduced, exhibited, published, distributed, and/or otherwise used individually or in conjunction with other media in any medium, including without limitation print publications, digital publications, and/or public broadcast for any lawful purpose. There is no time limit on the validity of such permission nor any geographic limitation on where these materials may be used.
If permitted, I transfer, assign, grant, and otherwise convey to Global Liver Institute and its employees, agents, affiliates, licensees, successors, and third-party organizations my entire right, title, and interest, if any, in and to all Copyright Materials—all ownership rights and the irrevocable right and permission to use, reproduce, copyright, publish, sell, distribute, and/or advertise or promote the recorded video, photo, interview, audio, and/or written content. Global Liver Institute has no obligation to create or use the Copyright Materials or to exercise any rights described herein.
My participation is voluntary and I may, at any time after signing, discontinue my involvement by notifying Global Liver Institute in writing. I understand that Global Liver Institute can see no present risk, and that I take full responsibility for my involvement in this project and any associated risks (whether legal, physical, mental, or emotional). To the fullest extent permitted by applicable law, I waive and release all legal and equitable rights relating to any liabilities, claims, demands, actions, suits, damages, and expenses of any kind whatsoever, arising directly or indirectly from Global Liver Institute’s use of the Copyright Materials.
By marking my answer below, I acknowledge that the foregoing is a binding agreement, and that I have completely read and fully understand the media release and its terms.
